In a responsive casualty: Call an ambulance. Reassure the casualty. Try and keep the casualty alert. If in doubt and consicious, give sugar.

The casualty’s attack is their first. The casualty’s condition is worsening and they are showing sings of exhaustion. You feel you are unable to cope with the situation. The casualty is incapable of any kind of speech.  The inhaler provides no sings  of relief after a few mins. The casualty does not have his/her medication/ inhaler.
